CONSENT 

At Asunta Cosmetic Tattoo, client safety, transparency, and informed decision-making are our top priorities. This Consent Policy ensures that all clients fully understand the nature of cosmetic tattoo procedures, potential risks, and their personal responsibilities before receiving services.


By booking an appointment and/or receiving services from Asunta Cosmetic Tattoo, you acknowledge and agree to the terms outlined below.

Client Eligibility & Acknowledgment

By consenting to services, you confirm that:

You are at least 18 years of age.
You have voluntarily chosen to receive cosmetic tattoo services.
You are not under the influence of alcohol or drugs at the time of treatment.
You understand cosmetic tattooing is a form of body modification, not a medical procedure.

Medical Disclosure Requirement

You acknowledge that you have truthfully disclosed all relevant health information, including but not limited to:

Pregnancy or breastfeeding
Autoimmune conditions or compromised immune system
Skin conditions, allergies, or sensitivities
History of cold sores (for lip blush services)
Use of Accutane, chemotherapy, steroids, or other medications affecting healing
Recent cosmetic procedures near the treatment area

Failure to disclose accurate information releases Asunta Cosmetic Tattoo from liability for adverse outcomes.

Understanding of Risks & Healing

You understand and accept that:

Cosmetic tattooing involves risks including redness, swelling, bruising, infection, allergic reactions, pigment migration, scarring, or uneven results.

Healing varies by individual and depends on skin type, lifestyle, immune response, and after-care compliance.

Results are not guaranteed, and final color, shape, and retention may vary.

Multiple sessions or touch-ups may be required to achieve desired results.

I understand that a sensitivity patch test is available to me upon request to test for allergic reactions to pigments or anesthetics. I understand that even with a negative patch test, an allergic reaction can occur later. By accepting this policy, I waive the patch test and release Asunta Cosmetic Tattoo from liability regarding allergic reactions.

Pre-Care & After-Care Responsibility

You acknowledge that:

You have received and understand pre-care and after-care instructions.

Failure to follow care instructions may result in compromised results or complications.

MRI Disclosure: I understand that the pigments used contain ingredients (such as iron oxides) that may affect MRI scans. I agree to inform my radiologist or MRI technician that I have cosmetic tattoos prior to any scan.

Procedure Consent

By providing consent, you authorize:

The artist to perform the agreed-upon cosmetic tattoo procedure.

Adjustments during the procedure based on professional judgment.

Photography & Media Consent

You acknowledge and agree that:

Before and after photos may be taken for documentation.

Images may be used for educational, marketing, social media, or portfolio purposes.

Your identity will not be disclosed without permission.

You may request in writing to opt out of photo usage prior to your appointment.

Touch-Ups & Maintenance

You understand that:

Cosmetic tattoos fade over time and require maintenance.

Touch-up appointments are not always included unless explicitly stated.

Touch-ups may incur additional fees.

No Guarantee & Release of Liability

You acknowledge and agree that:

No guarantees have been made regarding exact color, shape, or longevity.

You release Asunta Cosmetic Tattoo, its owner, artist, and affiliates from liability for outcomes resulting from:

Undisclosed medical conditions
Failure to follow care instructions
Natural healing variations
External factors such as sun exposure or skincare use

Digital & Written Consent

You understand that:

Consent may be provided electronically, verbally, or in written form.

Digital acknowledgment is legally binding.

Booking an appointment constitutes acceptance of this Consent Policy.
